We are currently transitioning to a new electronic medical records system, which will require all patients to create a new portal account.
The patient portal is our way of providing additional ways to communicate with our office. The patient portal is a secure online website.
Established patients may request a portal account by completing the information below. Our office will email you a link to begin creating your personalized account.
Minor Patients: At this time, parents and legal guardians may request access to a minor patient’s account. At this time, only certain features will be available for use. Requests for appointments, prescription refills and other medical-related needs must be made over the phone.
New Patients: You will not be able to request a portal account in advance of your first visit. Please let the Front Desk receptionist know if you would like an invitation at check-in.
